When you contact a particular organisation for assistance; they will make sure you are in the best place to get the help you need and that the right service is provided.

Most organisations will ask for some basic detail about your difficulty, and about yourself. You will also be asked if anyone else is already helping you with the difficulty; so whenever possible a joint plan can be agreed - to help you know who is doing what.

Most organisations need some very specific information to help them provide the right service. This information is different depending on what services are being provided. All professionals will need to record some information like your name, address, date of birth, and so on.

If you are writing to a particular organisation to get help, and they haven't already sent you a form to help you record the information they need, you may wish to use the form accessed from this page by clicking on the most appropriate link below.

When professionals fill out a form with you as part of the common approach to assessment within Lincolnshire, you will be shown a copy of what is recorded. You will normally be asked for your consent for the form to be submitted to another agency. Your consent can only be dispensed with under very specific circumstances, and these are outlined within the Information Sharing Between Professionals section of this site.
